As a youth care coach, I helped provide wraparound services to adopted children and youth experiencing significant health or behavioral challenges. I delivered in-home and community-based support focused on strengthening family relationships and improving day-to-day functioning. My team and I collaborated closely with families to prioritize family voice and choice, engaging them in goal-setting and decision-making processes. I supported youth in successfully navigating real-life situations by building coping skills, problem-solving abilities, and healthy communication patterns. We facilitated improved family interactions through consistent guidance, advocacy, and coordinated care.
